For this reason, below we have provided you with a HIPAA-compliant way to communicate patient information to our office through the internet, using our Online Referral Form below.

Online Referral Form

Our online Doctor Referral form allows you to communicate treatment requests much like paper referral forms. You may refer patients to our office by filling out our secure Online Referral Form.

Image files, Word documents, and PDFs can also be attached and sent with the Doctor Referral form. After you have completed the form, please make sure to press the Complete and Send button at the bottom of the page to automatically send us your referral form. You will then have the option to securely upload and transmit images of patient x-rays. Security of patient information is provided by PBHS.
